As a Memory Care Caregiver, you are responsible for enhancing the overall Resident experience through the provision of exceptional daily care and service in accordance with Resident's personalized care plan. This is to be handled in The Sterling Touchâ„¢ friendly, courteous, helpful, timely and professional manner, resulting in a very high level of Resident satisfaction. This position is under the general guidance of the Director of Health and Wellness in accordance with Community standards, processes, procedures, practices, and philosophy.


Essential Functions:

  • Participate in Start Sterling! line up each day.
  • Greet the Residents, guests, fellow employees, and vendors in a friendly and courteous manner.
  • Provide direct resident care as assigned and in accordance with Resident's personalized care plan to assist them with activities of daily living including dressing, eating, grooming, communicating, ambulation, toileting, and personal hygiene.
  • Participate in daily Grand Rounds communicating resident's changing needs and taking in information for flow of the daily activities.
  • Assist Resident's with daily activities in memory care and mobility needs, including but not limited to positioning and moving in and out of bed, chair, wheelchair as applicable as well as assisting them to prepare for activities, scheduled transportation to medical appointments, or scheduled outings.
  • Participation in planning and performing structured activities and spontaneous activity provisions according to resident preferences.
  • Guide residents through planned and spontaneous activity including social, recreational, fitness, life-long learning, and spiritual activities.
  • Observe and report changes to the nurse noted in resident's health, such as reports of pain, changes in vitals, weight, etc.
  • Observe and assist Resident in maintaining a safe environment within their residence.
  • Participate in ongoing community provided training programs to ensure state, federal and/or company requirements are met.
  • Take responsibility for uncompromising levels of cleanliness and creating a safe and accident-free environment in accordance with the protocols and the principles of Grand Living which includes cleaning the memory care neighborhood, resident apartments, changing beds, and providing laundry services for resident's personal laundry.
  • Provide support during dining hours with meal set up and break down as well as assist residents who need additional help while eating.
  • Supplement other health and wellness assistants/certified nursing assistants with Resident assignments as needed.

Other:

Employees are required to work a flexible schedule which includes, rotating weekend and holiday shifts.

Upon employment, all employees are required to fully comply with Grand Living's standards, procedures, and processes, including all government and industry regulations, for the safe and efficient operation of the Community. This includes regular attendance as well as wearing the prescribed uniform and maintaining a neat, clean, and well-groomed appearance. Employees who fail to meet the Grand Living expectations in all areas will be subject to disciplinary action, up to and including termination of employment.


SPECIFIC POSITION KNOWLEDGE, SKILL AND PHYSICAL REQUIREMENTS:

The individual must possess the following knowledge, skills, and abilities:

  • Ability to read, write and understand the English language in order to provide assistance with ADLs in accordance with each assigned resident's personalized care plan, observe and provide medication assistance to resident, react and communicate during emergencies, and communicate with other employees, supervisors, residents and guests.
  • Ability to stand, walk, climb stairs, crouch, bend, stoop, reach, grasp, push, pull and work continuously without sitting for extended periods of time.
  • Ability to lift or carry up to 40 pounds.

QUALIFICATIONS:


Education:

  • High School Diploma or G.E.D., required.

Experience:

  • One or more years of experience providing ADL's, highly preferred.
  • One or more years of experience working with seniors in a specialty dementia care setting, preferred.

Training:

  • Participation in state required dementia care training and continuing education requirements.
  • Participation in Grand Living dementia care specific trainings.

Schedule:

  • Part-Time Hours in Memory Care
  • Shift: 6:00 AM to 2:30 PM
  • Must be able to work weekends.
